Reporting

 

Case managers, adjusters, and treating physicians are welcome to attend our weekly Team Conference meetings on Tuesdays at 8 AM. Teleconferences can be provided if requested in advance. It will be necessary to coordinate visits or teleconferences with with us ahead of time.

All patients referred to our program will be thoroughly screened by Medical, Psychological and Exercise Therapy evaluations. An initial report will be provided to the referral source and carrier within 10 days of initial evaluation completion. The report will detail the patients' current diagnoses, and identify factors that have hindered progress in the past. A plan of treatment designed to move the patient towards recovery and optimal functioning will be formulated. Not all patients will be candidates for a functional restoration program. Those who do not meet our criteria will be referred back to their treating physician with a report outlining recommendations for treatment, and/or additional diagnostic work up.

Weekly status reports will be provided to the referral source and carrier via fax each week reflecting the patient's progress the prior week. A final report will be completed at the end of the program outlining the patients' progress and recommendations regarding the patient's status and future medical care needs. The vast majority of our patients will be permanent and stationary at the end of our program. The final determination is up to the referring provider. A formal rating report can be completed upon your request and authorization (not included in program costs). Case managers/ Adjusters/ Treating Physicians are welcome to attend weekly Team Conference meetings on Tuesdays at 8 AM.

Outcome studies will be completed on each patient focusing on improved functional ability, coping skills, reduction in health care usage, medications and return to work. Throughout and at program end, progress will be tracked using nationally recognized and peer reviewed measures. Follow up outcome studies will be provided for a period of up to 2 years when possible.
